All other students should use the checklist and forms for New Members.
The 2016 Physical Evaluation forms for Part A and Part B, included in the above packets, must be used.
DV will be hosting Sports Physicals for the 2016-17 year on Saturday, 9 April in the Small Gym from 8:30 to 12:30.
A Notary will be on staff.
Becky Richardson in the DV Athletics Office is now collecting completed forms. All forms must be turned in to Becky by 18 May.
Note: The $50 fee listed on the checklist cannot be paid until the new fiscal year starts in June.
