You can get your AIA Sports Physical at Desert Vista
Saturday, April 7 : 8:00 am – 12:00 noon : Small Gym
$15 cash or check (payable to Hedley Orthopaedics)
Bring Physical Packet w/pages 1-3 (evaluation) completed & SIGNED by parent/guardian
MUST have permission form if attending without parent/guardian

You can also go to your primary care doctor, or several clinics in the area also offer sports physicals.
TUHSD Requirement:
All MB and Guard students must complete the AIA clearance process before participating in any activities.
Sports physical and AIA paperwork must be submitted to DV (not the Band Boosters) by April 15.
Band registration is separate from AIA clearance.
