If you are NOT a marching band student, please complete the following forms.
If you ARE a marching band student, skip this – you have already done these forms.

Complete and submit online by Aug 15
Thunder Band Registration

Print, fill in, and return by Aug 15
Consent for Emergency Care form
~ must be notarized, or signed by DV front office Admin

First Quarter Video Scale Submission
