Tempe Union HS District athletes may attend a physical at any of the following dates/times:
April 3 – 2:30 pm – 4:30 pm @ Tempe HS
April 6 – 8:00 am – 12:00 noon @ Desert Vista HS
April 10 – 2:30 pm – 4:30 pm @ McClintock HS
April 17 – 2:30 pm – 4:30 pm @ Marcos de Niza HS
April 25 – 3:30 pm – 6:30 pm @ Mountain Pointe HS
May 11 – 9:00 pm – 12:00 noon @ Corona del Sol HS
Bring:
+ $15 cash or check made payable to Steward Orthopedic & Sports Medicine Center
+ AIA 2019 Pre-participation Physical Evaluation (3 page interactive pdf) completed, including athlete & parent/guardian signatures
+ AIA 2019 Pre-participation Physical Examination (1 page interactive pdf)
If coming without a parent/guardian, bring permission form:
+ Parental Permission for Sports Physical
There are no set appointment times. You will be seen in the order of arrival. Wait time will vary.
Doors will close at the end of time listed. You must be in line by closing time to receive a physical.
