2020 Banner Sports Med_Annual Physicals
AIA requires an annual Sports Physical – done after March 1.
Tempe Union will host Banner Sports Medicine for 1 day on each of the TUHSD campuses.
Cost
• $15 cash or check made payable to Banner Sports Medicine.
What to bring:
• AIA 2020-2021 Annual Pre-Participation Physical paperwork completed with parent/guardian signatures.
If coming without a parent/guardian:
• Please bring permission form.
There are no set appointment times. We will see you in the order that
you arrive. Wait time will vary.
Doors will shut at the end time listed. You must be in line by closing time
to receive a physical.
McClintock HS : Thursday, March 19th 3:00 pm – 5:00 pm
Desert Vista HS : Saturday, March 21st 8:00 am – 12:00 pm
Corona de Sol HS : Saturday, March 28th 8:00 am – 12:00 pm
Tempe HS : Thursday April 9th 3:30 pm – 5:30 pm
Marcos de Niza HS : Wednesday, April 22nd 2:15 pm – 4:15 pm
Mountain Pointe HS : Thursday, April 30th 3:30 pm – 6:30 pm
